The 12 Kingdoms, in essence, are on 5 islands in another 'dimension', if you will' then our own. They do not have live births, and all living things apparently grow from trees. If these 'soul fruits' get carried away some end up transporting to our world (more backstory on this in the series). And some creatures can go between both worlds.
The 12 Kingdoms were created by some superior power (called God, needless to say). Kings are made immortal, and they have a goody two shoes advisor. But even under these circumstances humans screw up, and bad things happen, thust he story line (the gods, although still existing, do not take much direct action - you'll have to watch the series for the exact details, which the anime does seem to provide)
Warning, the series doesn't end where its supposed to end, and one story arc is never completed. Something to do with its creator asking the anime to wait until he finishes/corrects/edits parts of it, which apparently he never got around to doing. Sad.
